As an Assertive Outreach Worker, you will play a critical role in engaging with individuals experiencing homelessness or at risk of homelessness in Shropshire. Your primary responsibility will be to provide support, guidance, and advocacy to help individuals access the services they need and work towards stable housing and improved well-being.

Responsibilities include, but are not limited to:

  • Actively engaging with individuals in the community, including those sleeping rough, in temporary accommodations, or facing housing insecurity.
  • Building trusting relationships to encourage participation in support services.
  • Conducting comprehensive assessments of clients’ needs, strengths, and barriers to housing stability.
  • Developing personalised support plans in collaboration with clients, focusing on their goals and aspirations.
  • Advocating for clients to access housing, healthcare, social services, and employment opportunities.
  • Providing practical support, such as helping clients complete forms and navigate complex systems.
  • Maintaining accurate and up-to-date records of client interactions and progress.
  • Evaluating the effectiveness of interventions and adjust support plans as necessary to achieve positive outcomes.
  • Responding to emergencies and crises promptly and effectively, providing immediate support and connecting clients with appropriate services.

Requirements:

  • Previous experience working in a similar role, such as outreach, social work, or community support.
  • Knowledge of issues related to homelessness, mental health, substance abuse, and social exclusion.
  • Strong understanding of local services and resources available to individuals experiencing homelessness.
  • Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with clients.
  • Ability to work independently and as part of a team, managing a diverse caseload effectively.
  • Willingness to complete an enhanced DBS if you do not currently hold one.
  • Full UK driving licence is essential.
