Job summary
We have an exciting opportunity to recruit anew permanent Specialist Mental Health Practitioners, to work in the North Bournemouth Primary Care Network . The role is funded through the Additional Roles Reimbursement Scheme (ARRS). Successful candidates must be experienced mental health practitioners and have experience of working in a Primary Care and/or Community Mental Health Team setting.
This is an exciting role for those who are proactive and passionate about innovation and providing high-quality care to people who have been identified as in need of mental health support in a primary care setting. The practitioners will access and link with secondary mental health services and help provide a "bridge" between primary care and secondary mental health providers. This will make it easier for people accessing services to obtain the help they need.
The practitioner will be embedded as part of the Primary Care Network working across the three practices which are part of the network. They will work alongside the multi-disciplinary team and be closely linked to the Community Mental Health Team and Steps to Wellbeing Service. The Practitioner will provide triage, assessment, interventions and lead on development and implementation of specialist mental health training in the practice.
Main duties of the jobThis role will work collaboratively between Primary Care and Secondary Mental Health Services as part of an Integrated Model of Care.
The intention is to focus services around local communities and local GP practices to help rebuild and reconnect healthcare teams across the communities they serve. This practitioner role will provide targeted care according to the needs of the population by supporting the PCN clinical strategy.
This role will work across an identified Primary Care Network to provide early assessment, treatment and improved access to appropriate care pathways and provide evidence-based interventions for people with mental illness.
The practitioner will facilitate onward access to other secondary care-based services, third sector services and link in with existing support services via the Primary Care Network.
