Chorley Central PCN is seeking a Clinical Pharmacist to enhance patient care and medication management. This role involves collaborating with GP practices, conducting medication reviews, and supporting public health initiatives. Candidates should possess a relevant pharmacy degree, registration, and experience in clinical settings.
Chorley Central PCN are recruiting a Clinical Pharmacist to support patient care in line with the Network Contract. The working days are to be agreed but the post will be 24 hours.
This is an excellent opportunity to join our existing team of 5 Pharmacists and 3 Pharmacy Technicians and work as part of our well established Network.
The successful candidate will provide clinical support on medicines optimisation and quality improvement.
The post holder will be responsible for reconciling clinical discharge summaries and where appropriate communicating changes to patients.

Clinical Pharmacist Job Description
The post holder will provide primary support to general practicestaff with regards to prescription and medication queries. They will helpsupport the repeat prescriptions system.
The post holder will work to integrate practices with communityand hospital pharmacies to help utilise skill mix, improve patient outcomes,ensure better access to healthcare and help manage workload. The role ispivotal to improving the quality of care and operational efficiencies sorequires motivation and passion to deliver excellent service within generalpractice.
Reconciliation of medications on discharge from hospital andother care facilities, implementing any monitoring or dose titration requiredincluding arranging blood tests and clinical review if required. Identify highrisk medications from discharge and ensure monitoring and prescribing is safeand consistent with Local Medicines Management Guidance. Pro-actively managepatients at high risk of medicines related problems and also address anydeviation from LSCMMG guidance, or other guidelines, by developing excellentworking relationships with hospital colleagues.
Conduct computer searches to identify patients at high risk ofharm from medicines. work with the patients and Practice teams to minimise harmby medicines optimisation.
Provide a telephone help line for patients with questions andconcerns about their medicines. Hold clinics for patients requiringface-to-face clinical medicines review with a focus on tacklinginappropriate polypharmacy. Undertake clinical medication reviews andreconciliation with patients with multi-morbidity and poly-pharmacy andimplement own prescribing changes and order relevant monitoring tests.
Make recommendations to GPs for medicine improvements andsupport multi-disciplinary teams.
To support public health campaigns and provide specialistknowledge on all public health programmes available to the public.
Answer all medicine related enquiries from GPs, other practicestaff and patients with queries about medicines.
Manage drug availability issues and medication queriespro-actively by liaising with community pharmacy colleagues and hospitalpharmacy colleagues. Develop effective working relationships.
Conduct clinical audits and improvement projects or work withcolleagues such as GP Trainees, Practice Managers, Practice Nurses etc
Contribute to the development of guidelines and procedures forGP practices across the Primary Care Network in the identified geographicalareas and negotiate adoption of these within the GP practices.
