Working in a fast-paced and highly pressured environment, you will manage and prioritise hospital discharge referrals, working swiftly and efficiently to arrange residential or nursing care placements that suit each individual's assessed needs. Working closely with local health and social care teams, providers, families, and individuals, you'll coordinate seamless transitions from hospital, ensuring people and their representatives understand their choices and the costs involved. Your role will also include supporting and managing contract and placement information.

This post is considered by KCC to be a customer-facing position.
