Job description

We are seeking an Administration Officer to join our Arranging Support Hospital Discharge Team, working to arrange essential services supporting people to return home safely from hospital. This is an exciting and pivotal role supporting people at every stage of their care journey.

About the Team:

The Hospital Team is responsible for coordinating and managing services for people leaving hospital - whether they require residential or nursing placements, or support at home through community services. We work closely with Locality Teams, Health professionals, providers, and families to ensure timely and effective transitions from hospital back into the community.

Key Responsibilities:

Be the first point of contact for the team: manage the team mailbox, handle queries and minor complaints, and direct calls to the appropriate staff.

Triage and allocate referrals, maintaining accurate trackers and up-to-date information on our systems.

Support the Hospital Team by assisting with the coordination of placements and care packages for people leaving hospital, covering both residential/nursing placements and community services.

Communicate effectively with individuals, families, service providers, and health and care professionals to ensure smooth transitions and ongoing support.

Maintain information and records in line with council policies, data protection, and record retention requirements.

Provide day-to-day administration and clerical support to the team, such as monitoring emails, ordering office supplies, and managing appointments and meetings (including minute-taking when required).

Process, maintain, and monitor financial records, including expense and income tracking, and resolving invoice queries., This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.

Previous experience in an administrative or customer service role is essential.
