Job summary

The Band 7 Nurse manager for the Anticoagulation Clinic will provide clinical and managerial leadership to ensure the delivery of safe, effective, and patient-centered anticoagulation service. The role requires effective working with the Lead nurse, consultants, pharmacists, haematology, cardiology, and primary care colleagues to ensure seamless patient care across the pathway. They will also contribute to clinical governance, service improvement, and training initiatives within the trust. The post holder will act as the clinical expert in the anticoagulation therapy, overseeing the care of patients prescribed warfarin, direct oral anticoagulants (DOACs), low molecular weight heparins, and bridging regimens within an outpatient clinic, and to provide support to community services and primary care. They will ensure patients receive appropriate initiation, monitoring, dose adjustment, education, and long-term management in line with national and local guidelines.

In addition to clinical responsibilities, the post holder will have full managerial accountability for the anticoagulation service team, including nurses and healthcare assistants. This includes workforce planning, rostering, recruitment, appraisals, training, and performance management.

Main duties of the job

• To effectively run nurse led outpatient's clinics alongside the Lead nurse.
• To initiate and follow up patients with Warfarin and DOACs and discharge safely to GP care.
• Work independently in the absence of a Lead nurse within clinical competency.
• To undertake teaching of patient's (carers if appropriate) in the treatment and management of the anticoagulation therapy medicines.
• Responsible for patient's safety and high standard of specialist care for all patients on anticoagulation therapy within the service.
• To liaise closely with specialist nurses and Lead nurse to ensure the effective triage of referrals to enable patients to access the appropriate element of the anticoagulation service.
• Responsible for contributing to specialty clinical governance group.
• To implement strategies to facilitate succession planning and retention of motivated staff.
• To set up clinics, maintain equipment and be responsible for clients and staff as laid down by Health and Safety regulations.
• Act as a role model for junior members of staff.
• Involving in helping in initiating DOACs and prevention of VTE (HAT).
• Work as part of the team.
• To be competent in the anticoagulation therapy software Helicon maintain accurate records of all patient encounters in line trust record keeping policy NMC.
• Undertake research projects in line with research skills and knowledge with view to presentation and publication.

Job responsibilities

Act as the clinical expert for anticoagulation management across the trust. Undertake comprehensive clinical assessments, care planning, and treatment of patients on anticoagulation therapy. Prescribe and adjust anticoagulation therapy (if a qualified non-medical prescriber) or provide recommendations to prescribers. Support patients and carers with education, self-management, and adherence to anticoagulation regimens. Lead on the management of complex patients and provide advice to multidisciplinary teams. Ensure the service delivers safe care in accordance with NICE, MHRA, NPSA, and local clinical governance standards. Ensure equitable, timely, and safe access to anticoagulation services for all patients. Act as patient advocate, ensuring care is personalized and patient centered. Maintain clinical oversight of caseloads across the anticoagulation service. Respond to complaints, incidents, and patient feedback, implanting learning and improvement. Ensure safeguarding procedures are embedded in the service delivery.

Develop, implement, and review local anticoagulation guidelines and standard operation procedures. Lead on policy development aligned with national and evidence-based practice. Contribute to Trust-wide strategic planning in hematology and long-term conditions management. Manage allocated service budgets efficiently, ensuring cost-effective use of resources. Authorize expenditure within delegated limits and maintain financial accountability. Ensure clinical equipment, INR monitors and digital platforms are used safely and maintained appropriately.

Person Specification

Education and qualifications

• Essential: Registered Nurse Degree Level, Anticoagulation course/ recognized evidence of education, qualification in Health Care Teaching Qualification or equivalent
• Desirable: Completed and attained the Non-medical prescriber course

Experience

• Essential: Substantial experience as an Anticoagulation Nurse with proven abilities, Substantial experience as Nursing Level Band 6 or above, Initiation of patients on DOAC and Warfarin, Complaints management knowledge
• Desirable: Experience of working with DAWN AC anticoagulation software
