A community healthcare provider in London is seeking motivated Anticoagulation Nurses to join their team. The role includes assessing, planning, and delivering patient care independently. The post holder will be expected to liaise appropriately with all members of the multidisciplinary team to provide an expert service to service areas related to the speciality. To be responsible for all aspects of assessment, planning, delivery and evaluation of direct patient care and function as an independent practitioner/clinical expert by virtue of their in-depth knowledge and experience. To support clinicians in the multidisciplinary team in providing clinical and managerial expertise in all aspects of the speciality ensuring the highest standard of nursing care. To lead, initiate and manage projects that will deliver enhanced patient care and inform best practice statements. To monitor and ensure a positive focused service incorporating patients' feedback to improve patient care. To act as an effective role model and mentor promoting excellence through practice.
