Job Summary

The post holder will be expected to liaise appropriately with all members of the multidisciplinary team to provide an expert service to service areas related to the speciality. To be responsible for all aspects of assessment, planning, delivery and evaluation of direct patient care and function as an independent practitioner/clinical expert by virtue of their in-depth knowledge and experience. To support clinicians in the multidisciplinary team in providing clinical and managerial expertise in all aspects of the speciality ensuring the highest standard of nursing care. To lead, initiate and manage projects that will deliver enhanced patient care and inform best practice statements. To monitor and ensure a positive focused service incorporating patients' feedback to improve patient care. To act as an effective role model and mentor promoting excellence through practice.

Main Duties of the Job
  • Always maintain a professional behaviour and promote a positive image of WF Federated GP Network, in line with World Class Care values.
  • To demonstrate clinical expertise.
  • To undertake clinics providing expert advice, counselling and individualised patient focused treatment and care.
  • To accept & prioritise referrals & autonomously manage a specific caseload without direct supervision, using own judgement & discretion.
  • Undertake such clinical procedures as necessary in the - management of individualised patient care, demonstrating a high standard of nursing practice.
  • To undertake all relevant procedures in accordance with WF Federated GP Network policies and ensure that these procedures and policies are understood and adhered by all staff, within own management responsibility.
  • To actively participate in the development of relevant evidence based clinical guidelines/policies relating to the speciality.
