Job overview

We have an opportunity to join the Anticoagulant Team at the Luton and Dunstable Hospital. We are looking for a qualified nurse to work 27 hours per week over the four days Tuesday to Friday. The post holder will mainly be based at Luton but may be required to work at Bedford if necessary. The post is for a Band 6 or a Band 7 depending on experience. The Band 7 post holder must have a Non-Medical prescribing qualification. There is an opportunity for the Band 6 post holder to progress to a Band 7 once the Non-Medical prescribing qualification has been achieved.

The successful applicant will be involved in the day to day management of patients’ anticoagulant care. Full training will be given.

Main duties of the job

The successful candidate will be involved in the day to day management of the anticoagulant clinics. This involves face to face and telephone consultations. This can involve initiation and discontinuation of anticoagulants and the dosing of warfarin / sinthrome. You will be expected to follow NICE Guidelines in relation to follow up treatment, and consult with appropriate health care professionals as necessary.

Detailed job description and main responsibilities

The post holder will:

• Be responsible for providing and delivering outpatient anticoagulant services clinics to patients, working as part of a multi-disciplinary team to educate colleagues, patients and their carers on how best to manage their condition. Conditions include Atrial Fibrillation (AF), Deep Vein Thrombosis (DVT), Pulmonary Embolism (PE).
• Treat directly referred suspected DVT and PE patients in the outpatient nurse led clinic at L&D Hospital.
• Maintain and monitor standards of care, to ensure patient safety and quality of care, working with Clinical Nurse Manager/Department Lead to identify and resolve failures in standards of care and the care environment to improve the patient experience.
• Educate patients regarding the reasons for anticoagulation and the side effects of treatment. Teach patients to self inject Low Molecular Weight Heparin as required.
• Support healthcare professionals across secondary and primary care with anticoagulant needs.
• Assessment, education and support of suitable patients to enable self-management of point of care testing with anticoagulant therapy using a structured training programme, encompassing quality and control management.
• Provision of training for colleagues across secondary and primary care with involvement in education programmes. To include the computer aided software programme, near patient testing and quality control.

Qualifications

• Current NMC Registration
• The relevant post qualification experience

Communication Skills

• Good written and verbal communication skills
• Effective communication at interview

Knowledge

• General Knowledge of anticoagulation and conditions requiring anticoagulation
• Knowledge / experience of InPhase
