Job summary

A leading health service provider in Wales is seeking an Ambulance Care Hospital Coordinator to manage the day-to-day transport provision for patients. This role involves coordinating patient journeys, ensuring high-quality service delivery, and acting as a key contact for health boards and patients. Ideal candidates should possess excellent planning skills, have a minimum of 5 GCSEs, and experience in monitoring healthcare services. Knowledge of Welsh is desirable. This is an office-based position located at the Princess of Wales Hospital in Bridgend.

Job description

Job summary: Responsible for the coordination / monitoring of all aspects of the day to day transport provision for patients visiting The Princess Of Wales Hospital.

We are looking for an individual who is self-motivated, organised, who can work as part of a team and can also work unsupervised.

The role will is predominately office based so computer and phone skills are essential.

To support the delivery of a customer-focused approach to the provision of Out Patients transport in Wales through the effective supervision of patient journeys.

Responsible for coordinating patient journey information and planning optimum patient journey and assisting day controllers/planning with the quality assurance of live oncology transport journeys.

Act as a main point of communication between oncology patients, oncology units and transport providers control teams throughout Wales.

Main duties of the job

The Ambulance Care Hospital Coordinator is responsible for supporting delivery of safe, high-quality care that meets patient needs and aligns with commissioner expectations.

The role will play a key part within the Journey Coordination Centre (JCC) and the Enhanced Hubs delivering a customer-focused service to patients and health boards ensuring a focused approach to non-emergency healthcare transport.

Key responsibilities include:

  • Acting as the first point of contact for health boards, hospital departments, clinics, patients, and discharge desks.
  • Coordinating patient journey information, with a focus on maintaining quality and performance standards.
  • Developing and maintaining effective relationships to support smooth service delivery.
  • Proactively resolving issues on and off site, with the ability to escalate when necessary.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Person Specification

Qualifications and Knowledge Essential:

  • 5 GCSE's at grade C or above including English/Welsh and maths or relevant experience.
  • Ability to travel between WAST and HB sites in a timely manner.
  • Experience working with statistical information for performance and quality monitoring purposes.

Experience Essential:

  • Experience of monitoring and scrutinising activity and performance.
  • Experience liaising with clinics and other colleagues.
  • Experience dealing with day-to-day issues that arise at a hospital level, ensuring particular attention to quality and performance standards.
  • Experience of dealing with and resolving complaints.

Skills and Attributes Essential:

  • Excellent planning and organisational skills to help prioritise own workloads, ensuring that deadlines are met.
  • Good negotiation skills.
  • Ability to coordinate activity and provide a voice between patients, the hospital site and Ambulance Care.
  • Customer care skills with the ability to deal with any issues and escalate where necessary
  • Be proactive in seeking and resolving issues.
