Job overview

Hounslow Community Recovery Service is an integrated health and social care service for adults identified with uni and multi-disciplinary recovery needs. The service will support independence by providing rehabilitation and reablement after an acute or chronic illness, injury, fall or change in life circumstances. It aims to restore & maximise function & prevent deterioration in physical, psychological and social functioning through rehabilitation and reablement.

The Community Recovery Service runs 7 days a week from 8am to 6pm and provides therapy for various pathways which include:

General Rehabilitation

Reablement Team (CRS+)

Discharge to Assess

The Integrated Discharge hub at West Middlesex University Hospital.

Hounslow Community Rehabilitation Team works across both intensive short-term and longer-term rehabilitation pathways for orthopedic and trauma patients, elderly patients with falls and those with complex medical and physical conditions.

Main duties of the job

This is an ideal post for an experienced and enthusiastic Band 5 or B6 Physiotherapist looking to further develop clinical knowledge and management skills within our multidisciplinary team. The successful applicant should be able to provide therapeutic interventions based on a problem solving and advanced clinical reasoning approach. Opportunities for in-service and external training will be provided. This job will allow the successful applicant to further develop a range of clinical skills in Community Rehabilitation. There will also be opportunity to participate in audits, quality improvement projects and evidence-based practice development activities. Travel is an integral part of the post so applicants should possess the ability and willingness to travel across the borough for work purposes.

Candidates should have valid HCPC registration & relevant post-graduate experience as a Band 5/6.

Person specification

Qualifications

  • Bachelor/Diploma in Physiotherapy or equivalent
  • HCPC registered
  • Evidence of personal / professional development
  • Membership of CSP
  • Membership of special interest group

Skills

  • Work within & understand national & trust policies (data protection, equal opportunities, health & safety / lone working)
  • Remain updated with professional practice, evidence based practice & new research
  • Ability to communicate clearly & concisely via written & spoken word.
  • Good theoretical & practical knowledge of adult & elderly care treatments & modalities
  • Ability to cope with stressful situations & with emotional or aggressive patients/carers
  • Understanding of clinical governance & its implications
  • Experience of teaching other Physiotherapy staff members, patients & other health professionals
  • Post graduate experience of 2 years minimum covering core skills in rehabilitation & management of complex conditions
  • Broad experience of orthopedics, medical, falls, cardio-respiratory & elderly care.
  • Specific experience in the specialty designated to the post - community
  • Experience of working as part of a multi-disciplinary team
  • Well established knowledge of assessment tools, & treatment techniques relevant to the client group
  • Experience of clinical education of junior staff & students
  • Experience of audit
