A healthcare provider in Oxford is seeking an Allied Health Care Professional to deliver evidence-based care for patients in the Acute Hub. The role involves autonomous clinical assessments, managing long-term conditions, and collaborating with healthcare teams to ensure effective patient care. Strong clinical decision-making and communication skills are essential. The position is open to qualified Advanced Nurse Practitioners, ACPs, or Paramedics.
Job Purpose: The post holder will consult with patients presenting for minor illness assessments face-to-face, over the phone, and through electronic means. The role focuses on delivering evidence-based care for patients within the Acute Hub, managing long-term conditions and minor illnesses, and promoting preventative healthcare. As an autonomous practitioner, you are responsible for the care delivered, demonstrating critical thinking and clinical decision-making skills. You will work collaboratively with the general practice team, especially GPs, to meet patient needs and support policy and procedures.
Key Relationships: Patients, GPs, Nurses, Allied Healthcare staff, Local GP Prescribing Lead, other practice and PCN staff, Community Pharmacists, Hospital Pharmacists, Community Matrons, and District Nurses.
