Job description

Allied Health Care Professional (ANP, ACP, Paramedic)

Job Purpose: The post holder will consult with patients presenting for minor illness assessments face-to-face, over the phone, and through electronic means. The role focuses on delivering evidence-based care for patients within the Acute Hub, managing long-term conditions and minor illnesses, and promoting preventative healthcare. As an autonomous practitioner, you are responsible for the care delivered, demonstrating critical thinking and clinical decision-making skills. You will work collaboratively with the general practice team, especially GPs, to meet patient needs and support policy and procedures.

Key Relationships: Patients, GPs, Nurses, Allied Healthcare staff, Local GP Prescribing Lead, other practice and PCN staff, Community Pharmacists, Hospital Pharmacists, Community Matrons, and District Nurses.

Key Responsibilities:

1. Clinical Practice: Provide autonomous clinical assessments, diagnoses (where possible), and treatments for adults and paediatrics presenting with minor illnesses. Support the Acute Hub Service, triage and manage e-consultations, prescribe and administer medications per protocols, and maintain accurate clinical records. Manage treatment plans for long-term conditions, conduct annual reviews, medication reviews, and initiate tests, following up on results. Collaborate with healthcare professionals to diagnose, monitor, and treat long-term conditions, supporting patient adherence and providing medication advice. Work within legal and policy frameworks concerning vulnerable populations and safeguarding. Develop personal clinical expertise and contribute to clinical service development and training within the practice.
2. Administrative Responsibilities: Participate in practice administrative tasks, be aware of clinical KPIs, ensure compliance with data protection, record consultations accurately, attend meetings, and facilitate effective communication within and across teams.
3. Communication: Communicate effectively with team members, patients, and carers, recognizing and responding to communication needs appropriately.
