Job description

To comprehensively assess patients, gaining information about their physical health, mental health, social issues, as well as their alcohol history to format a plan of care based on the patient's individual needs.

With the support and guidance of senior nurses, advise and support emergency department staff on admission criteria for patients with alcohol issues attending the department.

To liaise with and build strong professional relationships with other healthcare professionals both within the hospital and external agencies, to meet the needs of the patient.

To screen patients to identify those at risk from alcohol-related health and social problems (hazardous drinkers) and those whose health is being damaged by alcohol (harmful drinkers) and undertake appropriate interventions or referrals.

To undertake individual case management to provide immediate and on-going support to those patients who are unsuitable or unwilling to engage in community services.

To attend multidisciplinary meetings to discuss referrals, frequent attenders, or patients in treatment.

To make accurate assessments of the risk patients pose to themselves and others and take necessary action within Trust safeguarding guidelines.

To educate patients and their families on alcohol-related harms and deliver brief advice.

To provide training to Trust staff on screening and brief interventions, and to participate in the development and delivery of education programmes.

Main Duties of the Job

The band 5 Alcohol Care Team Nurse will be an integral member of the Alcohol Care Team, which provides care and treatment for patients with alcohol misuse issues. This nurse will assess patients who misuse alcohol and advise Trust staff on the care and management of acute alcohol-related issues and care plans.

They will participate in the development and delivery of education programmes for Trust staff in the specialty of alcohol misuse. The band 5 nurse will be responsible for supporting a caseload of individual patients who are not engaging in the community, which results in hospital admission and frequently attend Trust services.

Working Hours

Monday to Sunday, daytime hours dependent on service need.
