To provide outpatient mental/behavioral health psychotherapy to patients within the Affirming Care (GAC) Program. The Psychotherapist II- GAC Program is a member of the clinical practice team and works collaboratively with other team members in providing patient care. The role involves coordinating care for individual patients, supporting patients and families in self-management using evidence-based approaches, engaging in health center projects targeting specific patient population needs, and utilizing effective communication strategies, including health literacy approaches.
