We are looking for an enthusiastic and confident clinical pharmacist with experience of working in mental health to support the leadership and delivery of a three-month project to improve medicines management and optimisation in our four Home Treatment Teams based in Guildford, Chertsey, Frimley and Redhill. Successful completion of this project will inform a business case for regular Pharmacy Service provision.
We are looking for a self-motivated individual who will thrive on a challenge, with an understanding of the medicine’s optimisation issues within mental health. A wide range of experience and skills are required including excellent clinical, organisational, and communication skills, sound IT skills and the ability to work unsupervised and as part of a team. You will have a proven track record of successful implementation of medicines optimisation projects and have the vision to develop innovative solutions. Previous experience of working with community mental health services or home treatment teams would be advantageous.
This role will involve:
- Visit and assess our teams to establish staff competence and knowledge relating to medicines management, and supporting the teams within the busy environment they work in.
- Working closely with our HTT managers, Clinical Nurse Consultant, Clinicians and Pharmacy colleagues.
- Setting out and implementing a comprehensive plan incorporating training needs analysis, patient and staff evaluation and a review of any relevant HTT medicines charts and paperwork where appropriate and present a report of the main findings and recommendations to the relevant service leads.
- Providing limited support to the teams around medicines optimisation and enabling staff to better support individuals using the service in the context of prioritizing completion of this project.
A registered pharmacist with the General Pharmaceutical Council, the postholder must have significant post registration experience, and highly specialist knowledge acquired through an accredited post-graduate pharmacy qualification (minimum 2-year university Diploma level) or equivalent demonstrable experience.
