Job summary

University Hospitals Plymouth NHS Trust is seeking an enthusiastic Advanced Clinical Pharmacist to lead medicines optimisation across Community Services. The post holder will work across Community Hospitals within the Integrated Care directorate, managing long-term conditions, frailty and multimorbidity, with a focus on high-quality patient-centred care.

The role includes cross-site work at Mount Gould Hospital, Tavistock Hospital and South Hams Community Hospital, teaching and supervising

• Provide specialist clinical pharmacy services and advice to the clinical specialties.
• Work with the lead pharmacist on strategic development of clinical pharmacy services and define roles for pharmacists within Community Services.

We are looking for an enthusiastic and motivated Advanced Clinical Pharmacist with a passion for delivering high-quality, patient-centred care across Community Services. The post holder will work across our Community Hospitals as part of the Integrated Care directorate and play a key role in the management of long-term conditions, frailty, and multimorbidity, with a strong focus on medicines optimisation across community and intermediate care pathways. The role requires regular working and travel across UHP community sites, including Mount Gould Hospital, Tavistock Hospital and South Hams Community Hospital, in line with service requirements.

You will join a forward-thinking and highly respected team committed to service development, integration, and improving patient outcomes across care settings.

This is an exciting opportunity to develop advanced clinical expertise while playing a key role in delivering high-quality pharmacy services across the Community Hospitals.

You will be embedded within a proactive multidisciplinary team, working alongside clinicians, nurse specialists and allied health professionals to deliver outstanding patient care across the Peninsula.

• Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.*
• You will be responsible for the provision of a high-quality patient focused clinical pharmacy service to Integrated Care and Community Hospitals.
• Act as team leader for the ward-based pharmacy team overseeing community Hospitals as part of the integrated care directorate (ICT).
• Undertake teaching and clinical supervision of pharmacy staff and other healthcare employees.
• Provide specialist clinical pharmacy services and advice to the clinical specialities, demonstrating a high level of clinical effectiveness.
• Work closely with the lead pharmacist and associate chief pharmacist for clinical services on the strategic development of clinical pharmacy services and establish clearly defined roles for pharmacists within Community Services.

Candidates who meet the qualifications for this post but may not yet demonstrate the full knowledge or experience required at Band 8a will still be considered for this role as a Band 7 to Band 8a progression opportunity. The successful candidate will receive the necessary support to achieve the required progression milestones and demonstrate the expected level of practice before progressing to an advanced specialist Band 8a role.
