Job overview

The innovative Waltham Forest Integrated Stroke Pathway is delighted to be able to further develop the vital role of the Rehab Support Workers (RSW) in providing the intensive and client-led therapy to residents with stroke, with this advanced level role.

The Advanced RSW is a band 4 role, building on practical experience and personal expertise in Stroke care and self-management. The successful post holder will take a leadership and mentoring role to improve practical therapeutic provision and process efficiency along our seamless hospital to home journey. Following the appropriate competency training, they will be able to autonomously carry out assessments both in hospital and community and prescribe and provide simple equipment. This post holder will work as a key member of the pathway, improving communication and flow to reduce carer anxiety and enhance client safety alongside trained colleagues.

Previous experience of Stroke rehabilitation and community working is essential, but equally valued are care, compassion, and communication skills.

Main duties of the job

Supporting the Therapy team within the Stroke Recovery Unit and Community Stroke Team to provide patient-centered care of high quality:

  • Assist therapists in patient rehabilitation by carrying out treatment plans and progressing them within agreed frameworks.
  • Conduct patient assessments and treatments in various settings including wards, gyms, and patients' homes.
  • Undertake formal assessments and reporting, including access visits and cognitive and functional measures.
  • Facilitate stroke survivors and their carers in identifying personal self-management goals.
  • Work autonomously to support patients' rehabilitation plans using advanced communication and therapeutic handling skills.
  • Assist therapists in treatments involving complex therapeutic handling needs.
  • Lead the efficiency processes of timetabling and prioritization for Rehab Support Workers (RSWs).
  • Lead the delivery of therapist-prescribed group exercises and education programs.

Qualifications
