What will I be doing?

We are a hospital-based team supporting Somerset residents to return to their normal lives following hospital admission, aiming to help individuals return home if possible.

As an Advanced Practitioner, you will:

  • Have managerial and performance oversight of a team, tailoring solutions around individuals’ needs, working with families and health/community partners.
  • Support timely discharge processes in collaboration with Discharge Liaison Services.
  • Focus on the individual and their discharge process.
  • Use performance and hospital data to prioritize resources during escalation periods.
  • Utilize the intermediate care model to assess services with NHS and care providers locally, promoting staying in one’s own home.
  • Provide social care challenge to health colleagues regarding decision-making, planning, and legal compliance, under the direction of the Service Manager.
  • Work with community, health, and third sector partners through neighborhood hubs to improve outcomes, develop plans, and reduce readmissions.
  • Liaise with partners to improve service provision, team training, and recruitment.
  • Support admission avoidance initiatives.

What experience or qualifications are needed?

We support ongoing development, but successful candidates will:

  • Be a registered social worker with Social Work England, with a strong continuous professional development record.
  • Have experience at an advanced social work level.
  • Be autonomous in adult safeguarding decision-making.
  • Possess a strong sense of social justice and a strengths-based approach.
  • Have sound legal literacy in social care.
  • Have access to/use of a vehicle (essential).

Additional information

This is a Grade 9 role with a salary of £40,476 to £44,711 per year. Appointments are usually made at the bottom of the scale.
