1. Manage casework for hospitalized adults, ensuring smooth care coordination and discharge planning.
2. Assess needs, develop care plans, and facilitate transitions.
3. Maintain accurate records and compliance with standards.

Essential Requirements:

• Social Work England registration
• Eligibility to work in the UK
