Job summary

Imperial College Healthcare NHS Trust is seeking an Advanced Practitioner to deliver patient-centred care across Cardiology and Cardiac Sciences, working with Registered Nurses (NMC) or HCPC registered Allied Health Professionals.

You will autonomously assess, diagnose, manage a wide range of cardiac pathways, prescribe medications within your scope, and support bed management and patient flow across Cardiology and Cardiothoracic Services.

Qualifications

  • Open to Registered Nurses with NMC registration or Allied Health Professionals with HCPC registration.
  • Advanced/Intermediate Life Support Provider required.
  • Postgraduate Level 6+ education in a relevant subject is preferred.
  • Independent Non-Medical Prescribing Qualification desirable.
  • Master’s in Advanced Practice or working towards it.
  • Relevant post-registration qualification in Cardiology or Acute Care.

Join our Advanced Practitioner (AP) team and play a pivotal role in delivering high-quality, patient-centred care across Cardiology and Cardiac Sciences services.

This exciting opportunity is open to both Registered Nurses with NMC registration and Allied Health Professionals with HCPC registration, offering the chance to work at an advanced level of clinical practice within one of the UK's leading specialist cardiac centres.

In this role, you will autonomously assess, diagnose and manage patients across a wide range of cardiac pathways, including elective admissions, emergency admissions, transfers from referring hospitals and step-down patients from critical care. You will undertake advanced clinical assessment, formulate management plans, request and interpret diagnostic investigations, and prescribe medications within your scope of practice.

Working closely with consultants and the wider multidisciplinary team, you will support patients throughout their admission, intervention and discharge, ensuring delivery of evidence-based care and excellent patient outcomes.

The role is also central to the operational management of Cardiac Services. You will hold the Cardiac Bed Manager Bleep, coordinating admissions, transfers, repatriations and discharges across Cardiology and Cardiothoracic Services. Your advanced communication, leadership and clinical reasoning skills will be essential in maintaining safe and effective patient pathways across the service.

Main duties of the job
  • Work autonomously and collaboratively within the multidisciplinary team to assess, diagnose, treat and manage patients across Cardiology and Cardiac Sciences services.
  • The post holder will hold the Bed Manager bleep, coordinating patient flow across Cardiology and Cardiothoracic Services. This includes facilitating timely admissions, transfers, repatriations and discharges, ensuring effective utilisation of specialist cardiac beds, and working closely with site teams, consultants, referring hospitals and multidisciplinary colleagues to optimise capacity and support safe, efficient patient care pathways
  • The post holder will be responsible for managing admissions, transfers, discharges and step-down patients from critical care areas.
  • They will undertake advanced clinical assessment, request and interpret investigations, prescribe medications, formulate treatment plans and facilitate safe and timely discharge.
  • The role is pivotal in maintaining patient flow across the cardiac pathway, supporting transfers from referring hospitals and ensuring patients receive high-quality, evidence-based care throughout their admission and follow-up.

Person specification
  • Current professional registration with the Nursing and Midwifery Council (NMC) as a Registered Nurse or the Health and Care Professions Council (HCPC) as an Allied Health Professional.
  • Advanced or Intermediate Life Support Provider
  • Evidence of accredited postgraduate Level 6 or above education in a relevant subject (e.g. Physical Assessment, Clinical Examination, Clinical Reasoning).
  • Independent Non-Medical Prescribing Qualification.
  • Master’s degree in Advanced Practice (or working towards completion).
  • Degree relevant to professional registration.
  • Relevant post-registration specialist qualification in Cardiology, Cardiac Sciences or Acute Care.
  • Teaching and Assessing Qualification.
Experience
  • Recent and significant postregistration experience within Cardiology, Cardiac Surgery, Acute Medicine or a related speciality.
  • Excellent understanding and experience in the recognition and management of cardiac emergencies.
  • Experience in the management and treatment of cardiac patients.
  • Familiarity with NICE guidance and other relevant cardiac clinical standards.
  • Experience of bed management, patient flow coordination, capacity management and operational decisionmaking within acute hospital services.
  • Experience of coordinating admissions, transfers, repatriations and discharges across multiple clinical areas to support effective utilisation of inpatient beds and timely patient care.
Skills/knowledge/abilities
  • Evidence of applying research and evidence-based practice to clinical care.
  • Advanced clinical assessment and diagnostic reasoning skills
  • Ability to make complex decisions, prioritise workload and work effectively under pressure.
  • Awareness of cardiac data collection and reporting requirements.
