Adult Advanced Practitioner – Adult Social Care - £45 ph

  • Location: Taunton, Somerset
  • Pay Rate: £45 per hour
  • Contract Type: Full-time, Temporary (3–6 months initially, possible extension)

About the Role

Somerset is seeking a highly experienced and motivated Adult Advanced Practitioner to provide high-quality operational and professional leadership across a defined locality or neighborhood team within Adult Social Care.

You will play a key role in ensuring the delivery of person-centred, strengths-based practice that promotes independence, wellbeing, and effective use of community assets. The role requires strong practice leadership and advanced knowledge in complex casework, safeguarding, and care planning.

Key Responsibilities
  • Provide professional leadership and support to social workers and practitioners in a designated neighbourhood or specialist team
  • Oversee and contribute to the delivery of high-quality, person-centred services, ensuring individuals receive the right support at the right time
  • Support adults to remain independent by enhancing family and community support networks
  • Ensure timely hospital discharges and prevent admissions through proactive planning and multi-agency collaboration
  • Champion individual choice and control, enabling people to shape and manage their own support
  • Promote professional standards in line with the Care Act, ensuring robust assessments, reviews, and safeguarding practices
