Job description

We are seeking a dedicated and skilled Advanced Clinical Practitioner to join our healthcare team. The successful candidate will play a crucial role in providing high-quality patient care, working collaboratively with other healthcare professionals to ensure optimal health outcomes. This position requires a strong clinical background, excellent communication skills, and the ability to work autonomously while also being an integral part of a multidisciplinary team.

Main duties of the job

Duties

  • Conduct comprehensive assessments of patients to determine their health needs and develop appropriate care plans.
  • Provide advanced clinical care, including diagnosis and management of acute and chronic conditions.
  • Perform physical examinations, interpret diagnostic tests, and prescribe medications as necessary.
  • Collaborate with physicians, nurses, and other healthcare professionals to deliver coordinated patient care.
  • Educate patients and their families about health conditions, treatment options, and preventive measures.
  • Maintain accurate and up-to-date patient records in accordance with legal and ethical standards.
  • Participate in quality improvement initiatives and contribute to the development of clinical guidelines.
  • Stay current with advancements in medical practices and participate in ongoing professional development.
Job responsibilities

Key Responsibilities:

  • Provide advanced clinical assessment, diagnosis, treatment, and evaluation of patients across a broad range of conditions and ages.
  • Prescribe medications and treatments within scope of practice and according to local and national guidelines.
  • Undertake face-to-face, telephone, and online consultations, including home visits when required.
  • Work collaboratively with GPs, nurses, pharmacists, and the wider multidisciplinary team to ensure integrated patient care.
  • Triage patients and manage same-day demand efficiently.
  • Provide health education, disease prevention advice, and lifestyle support.
  • Contribute to clinical governance, audits, service improvement, and protocols.
  • Supervise and support junior staff and trainees where appropriate.
Person Specification
Qualifications
  • A relevant degree in nursing or allied health profession with advanced clinical training.
  • Registration with the appropriate regulatory body (e.g., NMC or HCPC).
  • Significant clinical experience in a relevant healthcare setting.
  • Strong assessment, diagnostic, and decision-making skills.
  • Excellent interpersonal and communication skills, with the ability to build rapport with patients and colleagues.
  • Ability to work independently as well as part of a team.
  • Proficiency in using electronic health records and other relevant software applications.
  • A commitment to continuous professional development and staying updated on best practices.
  • Experience using EMIS Web (or other clinical systems).
  • Familiarity with QOF and CQC standards.
