Overview

Are you an experienced social worker with a knack for handling complex cases, mentoring colleagues, and navigating the twists and turns of hospital discharge planning? Do you thrive in fast-paced environments where no two days are the same? If so, we need YOU in our Hospital Social Work Team. We are on the hunt for an Advanced Practitioner who can juggle high-level casework, lead reflective supervision, and provide expert guidance to our dynamic team. You\'ll be the go-to person for complex safeguarding decisions, legal frameworks, and multi-agency coordination. You\'ll also help shape services, mentor students, and champion specialist areas like Section 117 aftercare.

Responsibilities
  • Joining a close-knit, supportive team that is passionate about social work.
  • Coaching, mentoring, and inspiring colleagues, including newly qualified social workers and students.
  • Taking on a leadership role—informing practice, representing the team in key meetings, and stepping up in the absence of the Lead Practitioner.
  • Providing high-quality, complex casework across diverse settings (hospital wards, prisons, community settings, and more).
  • Ensuring complex situations are made manageable and supporting adults to achieve the best outcomes.
Qualifications
  • A qualified social worker with relevant experience in hospital social work or a similar fast-paced setting.
  • A deep understanding of safeguarding, mental capacity, and Care Act compliance.
  • The ability to balance leadership with hands-on casework, keeping the team motivated and practice-focused.
  • Strong mentoring and development skills—supervision, training, or acting as a role model.
  • Strong multi-agency working skills—liaising across multiple hospitals, councils, and health trusts.
  • Fluency Duty: In accordance with Part 7 of the Immigration Act 2016, the ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
