Job Overview:

RWJBarnabas Health, Primary Care Service Line is seeking Full-Time Advanced Practice Providers, Nurse Practitioners or Physician Assistants, to provide regional coverage within the primary care service line.

Qualifications:
  • Current NJ APN/PA license and certification required.
  • Master's degree or Graduate Level Certification required.
  • Certified Nurse Practitioner
  • CDS, DEA, and BLS required.
  • Proficient in database and reporting tools such as Microsoft Word, Excel, and PowerPoint.
Scheduling Requirements:
  • Monday-Friday, Day shift
  • Regional Onsite Coverage
Essential Functions:
  • Perform comprehensive initial and episodic health assessments to identify health problems.
  • Develop and implement individualized nursing and medical care plans in collaboration with the interdisciplinary team.
  • Initiate diagnostic, therapeutic, medical, and nursing actions with physicians.
  • Provide follow-up care, assess progress, and recognize emergent issues.
  • Monitor and adjust medications per protocol, and implement nutritional therapies as needed.
  • Triages patient calls requiring medical attention.
  • Order lab and diagnostic procedures; interpret findings.
  • Document all patient information accurately in charts.
  • Provide health education to patients and families.
  • Manage medical emergencies according to standing orders.
  • Ensure continuity of care across healthcare settings.
  • Offer counseling and referrals for psychological or social issues.
  • Serve as a resource and educator for healthcare team members.
  • Collaborate with partner sites to coordinate care.
  • Evaluate care quality and patient outcomes, and suggest interventions.
  • Travel as needed for onsite and telemedicine support within the regional territory, with mileage reimbursement.
