Overview

In collaboration with the Hospitalist, facilitates, coordinates, and implements the plan of care for hospital patients. Works with the interdisciplinary team to ensure optimal patient care outcomes.

Responsibilities
  1. Orders appropriate diagnostic tests and other related studies that are pertinent to the patients' plan of care.
  2. Follows up on pending diagnostic tests and evaluations in a timely manner.
  3. Communicates abnormal findings to the hospitalist on duty in a timely manner.
  4. Adjusts plan of care as necessary to achieve optimal patient care outcomes.
  5. Completes a thorough and timely assessment of patients.
  6. Communicates abnormal assessment findings to the hospitalist on duty.
  7. Collaborates with the hospitalist on duty to correct the plan of care as necessary to achieve optimal patient care outcomes.
  8. Provides patient education and discharge instructions to all assigned patients.
  9. Provides optimal customer service to patients and ensures plan of care is verbalized by patients prior to discharge.
  10. Communicates discharge plan of care with the Primary Care or Attending Physician of record.
  11. Provides accurate and timely documentation in the patients' medical record to include H&P, progress notes, and patient care orders.
  12. Follows up on previously dictated discharge summaries and relevant history and physical exams from previous visits or patient care encounters.
  13. Collaborates with the Hospitalist to ensure adequate and necessary resource utilization is being implemented for patients.
  14. Collaborates with the Hospitalist and UR Department to identify patients that are not meeting observation level criteria.
  15. Actively progresses these patients to inpatient status.
  16. Discharges patients as appropriate that do not meet observation level criteria.
  17. Actively participates in daily multidisciplinary rounds.
  18. In some instances, may lead rounds in the absence of the UR Physician or Hospitalist.
  19. Prepares for rounds and has a strong level of knowledge of the patients' plan of care.
  20. Integrates knowledge of pathophysiology into the discussion of multidisciplinary rounds and makes recommendations to alter the plan of care as necessary.
  21. Continuously revises and progresses the plan of care towards the discontinuation of observation status.
  22. Actively collaborates with the multidisciplinary team to ensure this happens in a timely manner.
Qualifications
  • NP - Nurse Practitioner
  • BLS - Basic Life Saving certification
