An exciting opportunity has arisen for a critical role as an Advanced Pharmacist in the East Hampshire Primary Mental health Care Team. The successful candidate will be at the forefront of informing and delivering a new way of working in mental health.
Working with Primary Care Networks within East Hampshire, you will help support patient pathways across the interface of primary and secondary care. You will work closely with general practice staff, mental health practitioners based in primary care as well as community mental health staff.
The post involves working alongside doctors, nurses and other healthcare professionals in both primary and secondary care to provide a more integrated health care provision. The ideal candidate will demonstrate a high level of clinical expertise, have excellent communication and interpersonal skills and need to be flexible, self driven and adaptable.

Support the development and work within the Primary and Community Mental Health Transformation programme pathways, protocols and guidelines so the patients receive safe, clinically effective and timely mental health medication in the most appropriate care setting.
Work with health and social care providers to promote existing and develop new tools, to increase the annual health checks in patients with Severe Mental Illness and other Mental Health conditions as indicated.
Support the transition of patients across care settings so they receive the required level of care to support them in taking their medication safely in the most appropriate care setting.
The post holder will work closely with identified Primary Care Networks
Support and influence GP based Clinical Pharmacists to ensure that the pharmaceutical needs of patients with mental illness are met through partnership working. This includes developing and supporting the implementation of effective communication systems across primary and secondary care.
To lead on providing specialist mental health medicines information and medicines optimisation support to health care professionals working in primary care network to help achieve the objectives of supporting people with significant mental illness to receive optimal care with medicines in primary care.
The post holder will be an integral part of the multi-disciplinary team (MDT) working with both Primary and Secondary Care.
