Job description

An exciting opportunity has arisen for a dynamic and innovative ANP to join NHS Fife’s Community Respiratory Team as an Advanced Nurse Practitioner (ANP) to support people living with Chronic Respiratory Conditions in the community.

The Community Respiratory Team currently operates Monday to Friday service with the potential of a 7-day service, including Public Holidays. The team are currently based within Whytemans Brae, Kirkcaldy, but travel and review patients across the whole of NHS Fife. The post holder will participate in the ongoing development, implementation and evaluation of the Community Respiratory Team, thereby improving the management, treatment and support of patients with chronic respiratory conditions, and their carers/families. The service aims to support self management, improve quality of life reduce unnecessary hospital admission / re-admission and support early discharge under the H@H plus model. CRT focus mainly on COPD, Bronchiectasis and ILD population. The service provides seamless care between the primary care and acute care settings.

The service is continuously undergoing service development and working with primary and secondary care to meet the needs of current respiratory drivers within NHS Scotland and Fife. This post would require a qualified ANP who is a proficient non-medical prescribing and has had respiratory experience prior to this role. The successful candidate would need to meet the needs of the changing service, which includes flexibility of working weekend and/or public holiday.

As this is Fife wide community post you will be required to reliably and efficiently travel whenever required across various sites/ bases/ and patient’s homes across Fife
