Birmingham and Solihull Mental Health NHS Foundation Trust is seeking a Band 7 Advanced Nurse Practitioner to join the North Birmingham Neighbourhood Mental Health Team, working with GP partners. The post is full time within our community services.

You will lead mental health triage, undertake assessments and deliver outcomes-based interventions, including nurse prescribing within the Primary Care Mental Health Team.

Responsabilités

Mental health triage, assessment and outcomes-based interventions for service users, including nurse prescribing.
Identify need and best course of advice, support or intervention.
Promote psychosocial wellbeing and pathways for service users and carers.
Ensure robust communication across BSOL NHS Foundation Trust and primary care, including participation within local neighbourhood MDTs.
