Job summary

Lion Health is recruiting an Advanced Nurse Practitioner to join the SWL PCN based at The Limes Medical Centre and supporting Chapel Street Surgery. The role involves assessing and treating a range of acute and chronic conditions within NHS ARRS guidelines, with advanced decision-making and independent judgement.

You will work across practices with GPs and allied staff, delivering patient-centred care, home visits when needed, and contributing to multidisciplinary care and career development

Qualifications

• Registered and experienced Advanced Nurse Practitioner with NHS prescribing rights.
• Experience in assessing and managing complex, chronic and acute conditions.
• Ability to work within local and national guidelines and protocols.

Responsibilities

• Provide expert clinical assessment, diagnosis, and treatment for a wide range of conditions.
• Conduct comprehensive health assessments for patients with complex needs.
• Undertake home visits when needed to support patients at home.
• Develop and evaluate patient management plans with the multidisciplinary team.
