Job description

Advanced Nurse Practitioner (Primary Care)

This is anexciting opportunity for an experienced Advanced Nurse Practitioner with aspecial interest in frailty/older peoples health to develop their clinicalpractice in a leadership role.

You will workdirectly with a small team including a community Paramedic and CareCo-ordinators to provide support to patients who are care home residents alongwith frail older people in their own homes.You will ensure that the PCN and member Practices meet theirrequirements to provide enhanced health in care homes as well as working withcare homes to develop and promote good practice to maximise the health andquality of life of their residents.

You will needto be registered as a Nurse with the Nursing and Midwifery Council, and hold anappropriate Advanced Practice qualification (post-grad diploma or degree, orMasters if qualified after 2020). You should also hold or be working towards aprescribing qualification.

Main duties of the job

The purpose ofthis role is to support PCN staff and member Practices to deliver high qualitycare to frail elderly people and residents in nursing and care homes. As an Advanced Practitioner the postholderjob plan will include research, education and leadership roles as well asdirect patient care.

You will be working with a group of five GP Practices, each of which offers high quality patient care delivered by a supportive team, and as part of a wider PCN team with Pharmacists, Physios and Social Prescribers.

We offer a competitive salary;£48,526 -£54,619 (qualified prescriber - dependent on experience in an ANP role) Candidateswho are working towards but have not yet completed their Prescribing Qualificationwould start on a salary of £43,806 - £47,672

WorkingHours: Up to full time 37.5 hours (minimum 22.5hours).

Workingpattern: Flexible within Network Standard Hours(Monday Friday 8.00 20.00 and occasional Saturday working)

Mainresponsibilities

Clinical

Leads and manages the Enhanced Health in Care Homesprogramme which includes:

oUndertaking weekly ward rounds

oSupporting person centred holistic assessments ofall patients admitted / readmitted toa care home within 7 days.

oEnsuring that all residents have a personalisedsupport care plan within 7 days of re/admission.

Develops and manages highly specialised programmesof care and care packages and provides highly specialised advice concerningcare normally beyond the scope of normal advance clinical practice e.g.establishes differential diagnoses, management/treatment plans and makesreferrals as appropriate. Makes fast track referrals.

Works autonomously and be accountable for his/ herprofessional actions.

Undertakes clinical nursing practice at an advancedlevel using expert knowledge and clinical skills to deliver holistic care toregistered patients.

Assesses patients with a range of acute, non-acute,and chronic medical conditions. To take history, conduct appropriate physicalexaminations, formulate differential diagnoses and carry through managementtreatment plans ensuring relevant follow-up.

Manages undifferentiated undiagnosed condition andidentify red flags and underlying serious pathology and take appropriateaction.

Uses complex decision making to inform thediagnosis, investigation, complete management of episodes of care within abroad scope of practice.

Actively take a personalised care approach andpopulation centred care approach to enable shared decision making with thepresenting person.

Where appropriate makes referrals to otherhealthcare professionals in primary and secondary care.

Prescribes medications as appropriate.

Maintains accurate record keeping in Emis Web.

Makes critical judgements of the highest order tosatisfy the expectations and demands of the job e.g., first contact withpatients presenting acutely and manages care in the presence and absence ofprotocols.

Makes decisions where precedents do not exist, whereappropriate without recourse to others e.g., direct referral of patients toHospital Consultants without prior reference to other medical colleagues.

Advises and supports others where standard protocolsdo not apply.

Works within professional guidelines and codes ofconduct

Visitspatients at home and within care homes as part of the wider clinical team.

Communicatecomplex and sensitive information concerning a patients medical conditioneffectively to patients/carers, recognising that sometimes there are barriersto understanding.

Fostersgood public relations by ensuring courtesy and discretion when dealing withpatients and their relatives.

Supportsand advises patients on the promotion of health, prevention of ill-health andthe self-management of their health within their limitations.

Recognisessituations that may be detrimental to the health and well-being of theindividual and act on the findings.

Ensuresthat care given is supported by the best available evidence and local policiesand procedures.

Maintainsprofessional competency whilst remaining current and in date for NMCrevalidation.

Education

Complete therelevant training to provide multi-professional clinical practice and CPD forother roles within Primary Care for example Community Paramedic, nurses, HCAsand Care Co-ordinators

The postholderwill link with the Training Hub and other colleagues across BNSSG to ensurethat Care Home staff are aware of local training opportunities.

The postholderwill organise at least one training session per year for PCN and Practice staffto update on issues relating to the care of frail elderly people.

Research

Connexus PCNhas an active research programme, and the postholder will have Continuous involvement in research/evaluationand/or development work which may be as part of one or more formal researchprogrammes or activities. In addition,they will:

Actively contribute to PCN/Service/Teamaudit/Research activity as required.

Instigate audit activity as required for new andexisting services.

Identify, apply and disseminate research findingsrelating to clinical practice.

Leadership

The postholderwill establish strong working relationships with nursing and management staffin care homes to encourage good clinical practice.

The postholderwill work with the GP lead for care homes in each of the five Practices toidentify and follow up opportunities for the improvement of care for frailelderly and care home residents.

The postholderwill provide clinical support and advice to colleagues including CareCo-ordinators and Community Paramedic.

  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.
  • Meets the standards for registered ANP working at advanced level.
Skills and Abilities

  • Comfortable working in a range of settings including GP surgeries, Care Homes and patients own home with minimum support and supervision.
