Job summary

Harwood Group Practice is seeking an Advanced Nurse Practitioner for a part-time role (30 hours/week) operating between 08:00 and 18:30, Monday to Friday. You will split time between the practice and home visits for housebound patients, ensuring continuity of care in the community.

You will work within a MDT with GPs and allied professionals, maintaining high standards of prescribing and clinical decision-making to improve outcomes. Independent prescribing rights are required.

Qualifications

• Qualified Advanced Nurse Practitioner with independent prescribing rights.
• Experience in both practice-based and community medicine.
• Strong triage, diagnostic, and prescribing skills.

Responsibilities

• Assess, diagnose, and treat acute and chronic conditions.
• Lead home visits for housebound patients and provide continuity of care.
• Collaborate with MDT including GPs and allied health professionals.
• Prescribe safely within scope and local formularies.
• Order and interpret tests (bloods, imaging) and act promptly on results.
• Engage in health promotion and opportunistic screening.
