Job description

We are proactively seeking a nurse practitioner/advanced nurse practitioner to provide high quality clinical care to our patients, seeing a variety of patients across the range of primary care. This position is offered on a part-time basis for 30-35 hours per week ideally working over 5 days.

The ideal candidate would be able to consult with patients and manage minor illness and acute disease presentations, as well as be knowledgeable about acute presentations of chronic diseases that would be commonly dealt with in the community such as asthma, COPD, hypertension and cardiac conditions. They would be able to work independently but would have collaborative support from the GPs and wider team. A culture of discussion and shared learning is encouraged. Holding an independent prescribing qualification is preferred but not mandatory.

They must have strong interpersonal and communication skills and be able to liaise and communicate effectively with the clinical and administrative staff within the practice and partnership.

An ability to demonstrate innovation and awareness of future opportunities and challenges would be desirable to help meet the current and future challenges of primary care, and to help us promote a rewarding environment for staff as well as ensuring excellent patient care. Main duties of the job
  • Diagnosing and treating patients presenting to primary care with acute medical conditions, minor illness and mental health problems, including a mix of face to face and telephone consultations.
  • Ensuring clinical practice is safe and effective and working within boundaries of competence.
  • Initiating and arranging appropriate investigations and referrals to other members of the primary and secondary health care teams as clinically indicated.
  • Participating in the administrative and professional responsibilities of the practice team including clinical audit.
  • Ensuring the clinical record is completed and maintained with accurate and contemporaneous details recorded.
  • Participating in the education and training of students of all disciplines and the induction of all members of the practice staff where appropriate.
