Advanced Nurse Practitioner - Mote & Wallis Primary Care Network

West Kent Primary Care (WKPC) is seeking to employ an Advanced Nurse Practitioner (ANP) on behalf of Mote & Wallis Primary Care Network (PCN) to work across their 3 member practices.

This role will support The MoteMedical Practice and Wallis Avenue Surgery in expanding capacity for patientsto access a clinical professional. The practices have a combined patientpopulation of around 17,000. The post holder will work across three sitesalongside the PCN Lead Pharmacist and PCN Pharmacy Technician. The PCN alsoprovides MSK and Childrens Social Prescribing services.

Main duties of the job

Mote & Wallis Primary Care Network (PCN) is looking for a caring,compassionate and experienced Advanced Nurse Practitioner (ANP) to join their PCN team. The ANP isa caring, compassionate, and experienced nurse who works within theirprofessional boundaries and scope of practice within the primary caresetting. The post holder appliesadvanced assessment and critical thinking skills to decision-making whentreating patients following initial history taking and clinical assessment todetermine a diagnosis, treatment, and treatment plan.

The Advanced Nurse Practitioner willprioritise and triage the needs of patients making any necessary referrals forinvestigations in line with local policies.The post holder will be required to communicate and work collaboratively with allstakeholders to meet the needs of patients, supporting the delivery of policyand procedures, and providing nurse leadership as required. They will focuson the four pillars of advanced practice: Clinical Practice, Leadership, Education,and Research.

Job responsibilities

Work as part of a multi-disciplinary team todeliver safe and high quality care.

Carries out clinical nursing practice at anadvanced level using expert clinical skills, critical judgements &knowledge to deliver appropriate and safe care.

  • Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with acute, long termand complex health needs.
  • Triages and treats patients, performingassessment of their needs and makes referrals as necessary to other members ofthe primary and secondary health care teams as appropriate.
  • Interprets investigation findings to determineeffective treatment plans and discusses with GPs where a second opinion isrequired
  • Proactively identify, diagnose and managetreatment plans for patients at risk of developing a long-term condition, usingboth non-pharmacological and pharmacological treatments.
  • Co-ordinate the planning and delivery of careensuring patients and carers or relatives are involved where required.
  • Prescribing and reviewing medication that isappropriate for patient needs and in accordance with latest evidence-basedpractice, national and practice protocols and within the scope of qualifiedpractice.
  • Diagnose and manage acute and chronicconditions, including drug and non-drug based treatment methods into amanagement plan.
  • Request diagnostic investigations and interpretwithin agreed protocols e.g. x-ray, blood tests, scans, etc.
  • Assess, diagnose and treat patients in practicepremises or their own home who require medical attention and/or refer toappropriate agency.
  • Refers patients to other members of themulti-disciplinary practice teams, secondary care organisations and otherorganisations as necessary.
