Advanced Nurse Practitioner - Independent Prescriber

Reports to:Clinical Lead/NonMedical Prescribing Lead

Responsibleto:Head ofPractice Clinical Services

Salary:Band 8.1 to 8.7 onthe ABC pay scale whichis equivalent to £50,901.77 - to £57,323.66

Hours of work: Fulltime/part time service coverage 08.00 to 20.00 Mondayto Sunday

Base:UKbased remote working. There will be aneed to regularly attend one of the ABCOffices in Surrey and Sussex

The post holder will be an experienced healthcarepractitioner with experience in acute and urgent care. They will have completedan MSc in Advanced Practice, including independent prescribing. The post holdermust be able to evidence a clinical portfolio and relevant competencies in linewith the HEE national framework for advanced clinical practice. Advancedclinical practitioners are expected to work across the four pillars of advancedpractice; clinical, leadership, research and education.

The post holder will practice as an Advanced NursePractitioner across various services, including but not limited to ABC RemoteServices (uCATS, Enhanced Access, Additional Primary Care)

Main duties of the job

The post holder will autonomously undertake assessment andtreatment of both acute and chronic, differentiated and undifferentiated,patient presentations.

The post holder will determine a clinical diagnosis,treatment plan and appropriate discharge, or refer, as indicated whilstmaintaining accurate patient records and ensuring patient safety. They will bea source of clinical expertise to less experienced staff, enhancing skills andknowledge, and work in partnership with a multidisciplinary team.

The post holder will ensure a high-quality patient centredservice is delivered in remote settings across all clinics.

Job responsibilities

Clinical responsibilities

  • Toindependently undertake specialised clinical assessments, diagnose, treat anddischarge, or appropriately refer, patients with a wide range of acute andchronic illnesses.
  • Toensure patients are fully involved in the design and implementation of theirtreatment plans.
  • Toensure effective coordination and liaison with multidisciplinary teams acrossprimary and secondary care, statutory and voluntary disciplines/groups toprovide a seamless service to patients and their carers and prevent hospitaladmission where urgent care is required.
  • To refer appropriately to clinicalleads/supervising GP where a second opinion is required.

Qualifications

  • NMC Registered healthcare professional
  • MSc in Advanced Clinical Practice
  • BLS
  • Evidence of CPD relevant to clinical area

  • Teaching qualification and/or up to date mentorship qualification
  • Completed, and continually updated, HEE Multi-professional framework for Advanced Clinical Practice

Experience

  • Significant and evidenced post registration experience (normally 8-years (5 years + 3 years for MSc)
  • Clinical experience of working autonomously within an appropriate setting including physiology, assessment, clinical reason and decision making.
  • Delivery of evidenced-based practice demonstrating a patient focused approach
  • Willingness to develop skills as needed by the service
  • Ensure clinical governance is embedded within practice
  • Awareness of own limitations and work within own scope of practice
