Our Frailty Service has gone from strength to strength, supporting Brownlows housebound population to achieve better health outcomes and wellbeing. We were proud to be shortlisted for Team of the Year in the 2024 GP Awards, and our Nursing team was awarded Nursing Team of the Year. We are now expanding our dedicated team and looking for a passionate professional to join us.

Role Overview:

As a key member of the Brownlow Health Frailty Team, you will spearhead the development of a new Acute Home Visiting Service, complementing our existing services. This innovative service will be supported by a multi-disciplinary team.

Main duties of the job

Key Responsibilities Will Include:

  • Acute and planned home visits for patients within the local Brownlow Health housebound population. These will typically be moderately/severely frail, living with Dementia or homeless, as well as refugee and Asylum seekers.
  • You will have experience in performing a holistic assessment and managing both acute health issues and long term conditions, referring patients to admission prevention services or secondary care as necessary whilst also collaborating with external services, patients and their families to create comprehensive care plans and follow-ups.

As an Advanced Nurse Practitioner, you will work in a nurse led multi-disciplinary team providing both acute and proactive planned home visits for our housebound patients.

The post holder will be able to demonstrate a sound knowledge in Advanced Nursing practice and the four pillars of practice. They must be an experienced non-medical nurse prescriber and used to working autonomously.

This is an exciting opportunity to support the development of an acute home visiting service within an already established service.

Clinical Responsibilities:

Provide acute and planned home visits for patients within the local Brownlow Health housebound population. These will typically be moderately/severely frail, living with dementia or homeless, as well as refugee and Asylum seekers.

Proactive management of Long- Term Conditions.

Conduct, as appropriate holistic assessments using the British Comprehensive Geriatric Assessment Tool.

  • Clinically examine and manage acute illness, including undifferentiated conditions and red flag presentations.
  • Use non-medical prescribing skills to initiate and manage appropriate treatment plans.
  • Screen for dementia, depression, and long-term conditions, and ensure timely onward referrals.
  • Complete medication reviews and optimise prescribing, with an awareness of anti-cholinergic burden.
  • Deliver immunisations during home visits (e.g., B12, Flu, Pneumonia, Shingles) to reduce follow-up appointments.
  • Facilitate care planning, including anticipatory care plans, DNACPR decisions, and GSF register inclusion.

Collaborative And Holistic Care:

  • Work closely with internal and external partners including GPs, Geriatricians, Mental Health teams, Community Matrons, District Nurses, Specialist nurses, Social Workers, Social Prescribers, health coaches etc.
  • Support informal carers and their health needs by liaising with external agencies and our inhouse Frailty Administrator.
  • Engage in safeguarding and MDT reviews for vulnerable patients.

Leadership And Management:

  • Take a leading role in the development of an acute home visiting service.
  • Mentor and support junior staff and team members; contribute if needed to monthly educational updates.
  • Lead or contribute to clinical audits, significant event reviews, and quality assurance processes.
  • Act as a role model within the nursing team, promoting clinical excellence and continuous learning.

Education Research And Development:

  • Support and participate in Brownlow Healths research projects and educational initiatives.
  • Collaborate with universities and public health bodies to embed current evidence-based practices.
  • Present findings, audits, and case studies in clinical meetings and contribute to policy development.

Such information must be considered strictly confidential and must not be discussed or disclosed. Failure to observe this confidentiality could lead to disciplinary action being taken against you.
