· GMS practice with GP-led community hospital next to the practice

· Well-maintained purpose-built practice

· Approx 2000 patients covering a geographical area of around 222 square miles

· Vision and Docman computer systems

· Cluster-linked FCP, Community Link worker, Community Nursing team, Primary Care pharmacy support, and Primary care Mental Health worker

· Locality-linked Midwifery and Health visiting teams

· High patient satisfaction scores in Health and Care Experience Surveys.
