Job description

Are you a senior nurse ready for your nextcareer step or challenge? We are looking for an enthusiastic and dynamic AdvancedNurse Practitioner to join our dedicated multi-disciplinary team in our state-of-the-artnewly redeveloped Inpatient Unit. This would suit someone who is motivated,compassionate, flexible and who has an interest in palliative care.

Therewill be opportunities to participate in education and audit, as well as deliverexcellent care to patients and their loved ones accessing hospice services.

Your role will be to work with the Medical Team and Palliative Care Consultants to deliver specialist palliative care to patients in our InpatientUnit and Wellbeing Centre, in a holistic manner, considering social, emotionaland spiritual health as well as physical symptoms. Our aim is to provideindividualised care, seeing the patient behind the diagnosis, so they canachieve their best possible quality of life.

Main duties of the job

Working as an independent practitioner and non-medicalprescriber the ANP is accountable and responsible for their own actions oromissions in line with the Nursing and Midwifery Councils Code and currentguidance on advanced nursing practice. You will always work within the scope ofprofessional practice and guidance on advanced practice, adhering to allrelevant professional, legal and regulatory guidelines.

You will assume responsibility for ownprofessional development and updates and maintain your own clinical skills asappropriate with support and guidance from the medical director and director ofclinical and patient services as required for the role. You will work withinthe boundaries of the role as it develops within Nightingale House and asagreed by the Medical Director and Director of Clinical and Patient Services inconjunction with the Chief Executive Officer.

There are opportunities for career developmentand progression to a nurse consultant depending on interest and experience.

Job responsibilities

Main responsibilities

Work autonomously to admit patients to theinpatient unit and care for them during their admission for end of life care,symptom management or respite.

Along with the rest of the medical team,nursing team and consultants, provide care for inpatients, outpatients andpatients attending wellbeing or rehab appointments if needed, including initialassessment, daily reviews, planning and management.

To identify and address the physical,psychological, spiritual and social needs of patients and the emotional,spiritual and psychological needs of their families and friends.

As an independent prescriber, prescribesmedicines for patients in accordance with local policies, national standardsand formularies relating to independent prescribing.

Work closely with and support the nursing teamand wider MDT, maintaining positive working relationships and close teamwork.

Communicate compassionately with patients andtheir families regarding their condition, treatment, prognosis and advancedcare planning.

Participate in clinical meetings as part ofthe MDT.

Maintain clinical records and completeadministrative tasks arising from medical work discharge summaries, GPletters following outpatient appointments, referral letters etc.

Identifies and reports issues and incidentsthat may affect the quality of palliative care services. Works with the team toensure the delivery of a safe and effective service.

Liaise closely with external partners suchas GPs, SPCT, district nurses, specialist clinical nurses and secondarycare.

Develop and maintain high professionalstandards within the service and promote multidisciplinary team working

To promote and participate in teaching andeducational activity, teaching nurses, medical students, GP registrars,colleagues, patients and families.

To participate in clinical governanceactivities e.g. quality improvement activities and ensure implementation ofbest practice guidelines.

Promote the ethos of audit in the organisationand identify, support and participate inclinical audits and evaluation of work to improve care and service delivery.

Ensure that effective governance arrangementsare implemented and maintained.

To maintain personal development throughpersonal development reviews, monthly 1:1 meetings, clinical supervision,training and appropriate courses, which the hospice will support.

Provide supervision and mentoring tonon-medical prescribers and trainee ANPs

There will be opportunities to develop aspecial interest or lead on programmes depending on skills and interests.

Lead the development of ideas and specialistpractice within the scope of the role. Innovates through exploring ideas withothers and takes action, thus contributing to different ways of working.

Develop services in conjunction with theservice leads, medical and clinical directors and wider team that will improveservices for users and contribute to the advancement of palliative care atlocal, regional and national levels.

Participates in maintaining the profile ofNightingale House Hospice through the presentation of quality improvementinitiatives at local, regional and national conferences as appropriate.

Work with the senior nursing team to completepersonal development reviews of the nursing team and provide support andmentoring.

Competence You areresponsible for limiting your actions to those that you feel competent toundertake. If you have any doubts about your competence during the courseof your duties, you should immediately speak to your line manager / supervisor.

Health and Safety Requirements of theHospice

All employees of the Hospice have a statutoryduty of care for their own personal safety and that of others who may beaffected by their acts or omissions. Employees are required to co-operatewith management to enable the Hospice to meet its own legal duties includingattendance at mandatory training updates and also to report any hazardoussituations or defective equipment.

The post holder must treat all information,whether corporate, staff or patient information, in a discreet, secure andconfidential manner in accordance with the provisions of the current dataprotection legislation and organisational policy. Any breach of suchconfidentiality is considered a serious disciplinary offence, which is liableto dismissal and / or prosecution under statutory legislation and the hospicesdisciplinary policy. This duty of confidence continues after the postholder leaves the organisation.

Records Management

As an employee of the hospice, the post holderis legally responsible for all records that they gather, create or use as partof their work within the organisation (including patient health, staff healthor injury, financial, personal and administrative), whether paper based or oncomputer. The post holder should consult the IG Lead if they have anydoubt as to the correct management of records with which they work.
