We are a fully merged PCN over three sites based in Fareham, Whiteley and Titchfield.

We are offering an excitingopportunity for the right individual to join our growingmulti-disciplinary clinicians. We arecurrently looking to expand and require an Advanced Nurse Practitioner/Advanced Paramedic Pratitioner todeliver same day care to individuals. This will include telephone triage, historytaking, clinical assessment, diagnosis, treatment, and evaluation of care. Youwill demonstrate safe, clinical decision making and expert care, includingassessment and diagnostic skills. Youwill demonstrate critical thinking in the clinical decision-making process

This vacancy is working part time 30 hours per week on a Monday, Tuesday and Wednesday.

Main duties of the job

Act as Advanced Nurse Practitioner/Advanced Paramedic Practitoner, demonstrating advancedclinical competence and a knowledge base beyond those associated withconventional nursing roles.

Work as an autonomous practitioner to undertake consultations,using own clinical judgement to diagnose, treat or refer patients and provideclinical case management for same day access.

Diagnose and manage both acute and chronic conditions, integratingboth drug and non-drug-based treatment methods into a management plan.

Ensure compete and accurate documentation of each and everypatient contact.

Make direct referrals to primary, secondary, and social serviceswithin locally agreed pathways, guidance and protocols.

Support patients to adopt health promotion strategies that promotehealthy lifestyles and apply principles of self-care.

Job Description

As an Advanced Nurse Practitioner (ANP)-Advanced Paramedic Partitioner you will be an experienced clinician who, acting within your professional boundaries, will provide same day care for patients registered at Meon Health Practice. This will include telephone triage, initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. You will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills. You will demonstrate critical thinking in the clinical decision-making process.

KEY RESPONSIBILITIES

Clinical Care

Act as Advanced Nurse Practitioner-Advanced Paramedic Practitioner demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing/paramedic roles.

Undertake consultations with patients as an autonomous practitioner and using own clinical judgement to diagnose, treat or refer patients.

Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug-based treatment methods into a management plan.

Make direct referrals to primary, secondary, and social services within locally agreed pathways, guidance, and protocols.

Work with patients and their carers in order to support compliance with and adherence to prescribed treatments.

Ensure care plans are in place, including emergency health care plans following discussion with the patient and family members.

Ensure that patients receive high quality clinical care, delivered in a timely manner.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs.

Ensure complete and accurate documentation of each and every patient contact.

Person Specification

Experience

• The right candidate will have a strong desire to achieve the best possible outcomes for patients in the community setting and have the motivation and flexibility to help develop the services as the team evolves to meet the needs of the population.
• The post holder will be a registered Nurse/Advanced Paramedic with post-graduate qualifications relevant to the position e.g. Physical Assessment & History Taking & prescribing.
• Primary care experience is not essential; however, a good understanding of how Primary Care works would be beneficial.
• Evidence of continuous professional development
