Advanced Nurse Practitioner / Advanced Clinical Practitioner

Hours: 25-35 negotiable

Description: We are looking for an Advanced Nurse Practitioner to join our Nurse- led, innovative, friendly, supportive General Practice in Hanwell W7. We are based in a modernised purpose built premises. We have been awarded a CQC outstanding status and General Practice award ‘Nursing team of the year’.

This is an exciting opportunity for the right person to join our team to deliver exceptional services to our patients. To work with multidisciplinary team with support of Nurse Practitioner Director, Nursing team, GPs and administrative team. They can also lead on modernising patient pathways, supporting autonomous clinical decision-making, and enhancing care delivery across a progressive, community-focused practice.

• Strong leadership, communication, and decision-making skills
