Advanced Nurse Practitioner / Advanced Clinical Practitioner

We are looking for an Advanced Nurse Practitioner to join our nurse-led, innovative, friendly, supportive general practice in Hanwell W7. We are based in a modernised, purpose-built premises. We have been awarded a CQC outstanding status and a General Practice award for Nursing team of the year.

This is an exciting opportunity for the right person to join our team to deliver exceptional services to our patients. You will work with a multidisciplinary team, supported by the Nurse Practitioner Director, Nursing team, GPs, and administrative staff.

Main duties of the job

Lead on modernising patient pathways, supporting autonomous clinical decision-making, and enhancing care delivery across a progressive, community-focused practice.

Job responsibilities

Job Summary:

  • Provide Nurse Practitioner services, including diagnosis, treatment, screening, and advice within professional competence.
  • Provide clinical leadership, including addressing Clinical Governance issues, urgent and routine results, correspondence, and instructions from external agencies such as hospitals and consultants.
  • Work independently in the absence of a GP, demonstrating initiative and confidence.
  • Collaborate as part of the Primary Healthcare Team, sharing decision-making and information appropriately.
