Responsibilities

  • Provide education and clinical assessment to patients within the virtual ward.
  • Monitor patient vital signs 3-4 times daily.
  • Document patient interactions and clinical decisions.

Job description

Fixed Term Contract - 12 months

26 hours per week

Tuesdays: 12pm-8pm

Wednesdays: 8am-12pm

Thursdays: 8am-12pm

Alternate Saturdays: 12pm-8pm

Alternate Sundays: 8am-8pm

Location – Remote

£58,000 per annum (pro rata based on a 40/hr week)

Reporting to: Service Delivery Team

Start date: ASAP

Holiday allowance: 25 days + bank holidays

We are excited to introduce a new virtual ward service designed to deliver coordinated healthcare to patients in the comfort of their homes, replicating the support and monitoring traditionally provided in a hospital ward. As an Advanced Nurse Practitioner (ANP), you will play a crucial role in this service, working remotely to engage with, assess, and support patients.

Responsibilities

·Patient Engagement and Assessment: Provide education, support, and clinical assessment to patients within the virtual ward.

·Vital Sign Monitoring: Oversee and monitor patient vital signs (e.g., heart rate, blood pressure, temperature) that are automatically uploaded to our platform 3-4 times daily.

·Clinical Decision Making:Use clinical expertise to interpret vital signs, make informed decisions, and escalate care when necessary.

·Patient Education: Educate patients on managing their conditions and using remote monitoring equipment effectively.

·Documentation:Maintain accurate and timely records of patient interactions and clinical decisions.

·Collaboration:Work closely with a multidisciplinary team to provide coordinated and comprehensive care.

Essential Requirements

· Must be a registered NMC nurse

· Must have an MSC in Advanced Practice or be RCN Accredited

· Experienced in dealing with patients with respiratory conditions including asthma, COPD and pneumonia
