Job summary

East Suffolk and North Essex NHS Foundation Trust is seeking an experienced Nurse Practitioner to join the Urgent Community Response Service (UCRS). You will deliver urgent care across patients' homes and care home settings, assessing and treating with autonomous prescribing.

You will work as part of a multidisciplinary team to prevent hospital admissions and support recovery at home. In this role you will undertake rapid assessments within two hours of referral, manage frailty and complex

The Urgent Community Response Service (UCRS) at ESNEFT is seeking a highly motivated and skilled nurse to join our dynamic and innovative team. This is an exciting opportunity to deliver high-quality urgent care in patients' homes and care home settings, helping people receive the right care at the right time while avoiding unnecessary hospital admissions.

Working within our multidisciplinary team, you will use your advanced clinical skills to assess, diagnose, prescribe, treat and manage patients with a wide range of urgent healthcare needs. No two days are the same, making this an ideal role for an experienced clinician who thrives on autonomy, clinical decision-making and providing outstanding patient-centred care.
