Job description

We are looking to appoint a highly motivated and committed Advanced Nurse Practitioner to join our senior clinical team primarily based at Moorcroft Medical Centre and branch site Moss Green Surgery, Stoke on Trent.

This is a permanent post, 22.5 hours per week. Please note there may be an occasional requirement to provide cross cover at our other integrated practice sites (Holmcroft Surgery, Stafford and Keele Practice, Newcastle Under Lyme).

This is a pivotal post in the delivery of quality and innovative primary care services to our patients. We are a friendly and well-organised team with a forward-thinking approach.

We are a high achieving, patient-centred Primary Care Directorate and have implemented a care model to improve patient experience, providing 15-minute urgent and routine appointments. Standard clinical sessions and the provision of administration time for non-patient facing work within the session is routinely included.

Main duties of the job

You will demonstrate outstanding autonomous and critical thinking and be a caring, compassionate and committed experienced nurse. You will, whilst acting within your professional boundaries, provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

You will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnosis, for patients within the practice. You will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
