Job description

This is an exiting opportunity for an experienced Advanced Nurse Practitioner to join Mayflower Medical Group which has five surgeries in Plymouth serving a population of over 33,000 patients. The successful candidate will provide routine clinics morning and afternoon and will respond to the urgent clinical task group and other queries generated throughout the day alongside other members of the team.

Part time hours will be considered.

Main duties of the job

A suitably qualified Advanced Nurse Practitioner with sufficient experience to work autonomously and independently who will:

1. Provide a morning and afternoon clinic for patients with undifferentiated and diagnosed health problems

2.. Respond to urgent clinical tasks and queries generated throughout the day

3. Take part in clinical meetings to share best clinical practice and learning

4. Play an active role in the development of the organisation, including quality improvement and patient safety

5. Work within NMC Code of Conduct and to agreed practice standards and protocols.

Job responsibilities

Summary

To provide care to patients at the point of entry to the health care system, including those with undifferentiated and undiagnosed health problems

To provide continuing care to patients across the whole practice population

To provide health maintenance and health promotional services to the well, offering educational supportive setting for patients to explore their health problems

To provide psychosocial as well as physical care to the practice population

To manage urgent tasks and queries received throughout the day

Specific areas

To work as an autonomous clinical practitioner accountable for own professional judgement and actions

Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a patient management plan

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within the scope of practice#

Undertake face to face and telephone consultations for patients presenting with an undifferentiated diagnosis to include consulting, assessing, diagnosing, implementing and evaluating treatment and interventions such as referrals and prescribing

Request and interpret tests appropriate to patients needs

Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care

Assess, identify and refer patients presenting with mental health needs in accordance with the national and local guidelines for Mental Health services

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment

Communicate with and support patients who are receiving difficult or complex information or results in a professionally appropriate and supportive manner

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

Person Specification
Qualifications
  • Clinical Advanced Practice to postgraduate level (postgraduate certificate or diploma, or full masters)
  • Nursing Degree
  • Chronic disease management qualifications
  • Sexual health and family planning qualification
Experience
  • Advanced clinical practice skills
  • Experience working independently and autonomously without immediate GP support (required for new models of care within a multidisciplinary team setting)
  • Experience consulting and prescribing for paediatric and adult patients presenting with undifferentiated and undiagnosed problems
  • Experience in carrying out home visits
  • Experience in training, mentoring and supervision
  • Experience in family planning and sexual health
  • Experience in managing mental health issues
