Job description

  • You MUST have a past experience of working as ANP in a GP surgery in UK
  • You will be responsible for attending to patients who wish to be seen bya health care professional; diagnosing and treating patients presentingwith minor illness seeing and advising people in respect of theircontinuing medical and nursing needs.
  • Prioritising and triaging the needs ofpatients accordingly, making any necessary referrals for investigations inthe appropriate manner.
  • Working withreference to practice agreed protocols and guidelines, providing nursingtreatments to patients both independently and in participation with nursecolleagues and general practitioners.
Main duties of the job
  • Provides patients with access toa Nurse Practitioner in surgery and over the phone for all presentationsin primary care.
  • Controls admission and dischargeto own caseload through:
  • Appropriatecomprehensive assessment to include physical, mental, emotional andsocial health including the use of invasive and non-invasiveinvestigations to establish a diagnosis.
  • Prescriptionof evidence-based care including medication.
  • Developingwith the patient and family when appropriate, an ongoing plan for healthwith an emphasis on prevention and self-care.
  • Follow-upand evaluation of care.
  • Directreferral to and liaison with consultants and other professional agencies.
  • Making andaccepting referrals within the practice team.
  • Ifqualified to, prescribes according to formulary and competencies.
  • Supportpatients to adopt health promotion strategies that promote healthylifestyles, and apply principles of self-care.
  • Support andmange health needs of women presenting for family planning, or sexualhealth consultation.
  • Assess,identify and refer patients presenting with mental health needs inaccordance with the NSF for Mental Health.
  • Acollaborative, supportive and holistic approach.
  • To keepaccurate patient records on computer system.
About us

The Practice has a list size of 11,500 patients. The Practice Team consists of 2 GP Partners, 4 Salaried GPs, a Nursing Team of ANPs, HCAs, Pharmacy Team and a well-established administration team. We inhabit modern premises which are located in Serpentine Green Centre, Hampton, Peterborough, where there is good access to shops, sporting facilites and restaurants. There is plenty of secure, free parking to the rear of the Practice.

Pathologicalspecimens and investigatory procedures

  • Undertakesthe collection of pathological specimens including intravenous bloodsamples, swabs etc. Perform investigatory procedures requested by the GPs.
  • Checks andprocesses results received via SystmOne as instructed by the GPs.

AdministrativeResponsibilities

  • Disseminates learning and information gained to other teammembers in order to share good practice and inform others about currentand future developments/courses.
  • Manages and assess risk within areas of responsibility,ensuring adequate measures are in place to protect staff and patients.
  • Participates in the delivery Chronic Disease Management and inachieving QOF Targets.
  • Ensures good communication between you and the team and othermembers of staff and communicate any changes of ways of working to thoseaffected as appropriate.
  • Attends and contributes to Practice Clinical Governance.
  • Encourages and ensure team working within the practice
  • To participate in practice clinical audits if required. Actsas a resource to other practice members.
  • Uses technology and appropriate software as an aid tomanagement in planning, implementation and monitoring of care, presentingand communicating information.
  • Reviews and processes data using accurate Read codes in orderto ensure easy and accurate information retrieval for monitoring and auditprocesses.
  • To participate in developing protocols and clinicalguidelines for use in the practice.
Person Specification
Qualifications
  • Registered General Nurse (Currently registered with the Nursing & Midwifery Council)
  • Recognised ANP qualification at minimum of degree level or equivalent
Experience
  • Evidence of appropriate continuing professional development in all aspects of the ANP role
  • Experience in management long term conditions
  • Evidence of working autonomously and as part of a team
  • Proven ability to evaluate the safety and effectiveness of their own clinical practice
  • Experience of minor illness, clinical management and patient referrals
  • Several years post registration experience
  • Several years experience working in General Practice
