The Avenue Surgery is recruiting an Advanced Nurse Practitioner to work three days a week (Monday, Tuesday and Friday) within the Leigh Primary Care Network. You will assess, diagnose and treat patients with acute and chronic conditions, from history taking through to evaluation, with referrals to specialists when needed.

This role suits an autonomous nurse with primary care experience who can contribute to service development within the practice and network, and who holds an ANP qualification

Qualificações

• Advanced Nurse Practitioner qualification with prescribing.
• Experience in primary care settings is essential.
• Ability to work autonomously within a multidisciplinary team.

Responsabilidades

• Provide care from initial history taking through clinical assessment, diagnosis, treatment and evaluation.
• Make safe clinical decisions and deliver expert care, including assessment and diagnostic skills.
• Manage a range of acute and chronic conditions within scope of practice.
• Assess, diagnose, treat and refer patients as appropriate.
• Work autonomously and alongside the multidisciplinary team.
• Use diagnostic and screening services.
• Work with colleagues in the same area of practice, and refer or share care with specialist colleagues in primary and secondary care.
• Help develop primary care services for the practice population and the Network.
