Join a dynamic clinical team as an Advanced Nurse Practitioner within the Coastal Medical Partnership, based at New Milton Health Centre. You will be responsible for diagnosing and treating minor illnesses, performing triage, and providing healthcare education, contributing to the delivery of high-quality patient care across multiple sites.
Coastal Medical Partnership is looking for an enthusiastic and motivated Advanced Nurse Practitioner to join our busy clinical team. The successful candidate will be an independent prescriber, with experience of undertaking home visits, clinical triage and providing Same Day Care.
This role includes attending to patients who wish to beseen by a health care professional; diagnosing and treating patients presentingwith minor illness; seeing and advising people in respect of their continuingmedical and nursing needs. Working alongside the Drs and/or Urgent Care Team toprovide same day telephone triage, face-to-face consultations or home visits. Theremay a requirement to see extra or emergency patients as appropriate. The ANP will need to prioritise and triage the needs of patientsaccordingly, making any necessary referrals for investigations in theappropriate manner.
You will work in our Same Day Hub based at New Milton Health Centre, and may undertake booked clinics at any of our four sites.

Job summary:
