A national health services provider is seeking an Advanced Nurse Practitioner for 12 months maternity cover, with the potential for a permanent role. This role involves patient assessment via various methods and requires relevant qualifications. Ideal candidates will have experience in managing acute as well as chronic health conditions. The position offers flexible working hours with the team support and educational opportunities.
An opportunity has arisen for an Advanced Nurse Practitioner tojoin our friendly supportive clinical team for 12 months maternity cover, possibly with a view to a permanent role there after.
We are recruiting for 20-30 hours a week.
The successful candidatewill be a registered nurse, qualified independent prescriber and have generalpractice experience. The role will involve assessment and management of patientsrequesting same day appointments, via telephone consultation, online access andseeing patients face to face.
Thepost-holder is an Advanced NursePractitioner(ANP)
Theyhave been assessed as competent in practice using their expert clinicalknowledge and skills. They have the freedom and authority to act and makeautonomous decisions in the assessment, diagnosis and treatment ofpatients.
TheANP provides an opportunity for patients to avoid the need to be referred to aGP, thereby enabling them to receive timely care and negate unnecessary delayin receiving treatment.
ANPs in primary care can develop close,long-term relationships with their patients and work in partnership with themto achieve optimum health. They are autonomous in making decisions based onassessment, diagnosis and the interpretation of test results. They canindependently prescribe appropriate medication, evaluate or refer to otherspecialists if necessary.
We are a semi-rural, dispensing, training practice set in beautiful Alresford, serving a population of 10,000 patients. We are spoilt with the area and population we provide healthcare for.
The practice has four partners, four salaried GPs, two retained GPs and an experienced Advanced Nurse Practitioner. We are proud of how we support and communicate with each other.
Our three practice nurses, one nurse associate & one health care assistant have years of experience and skills in treatment room, travel, womans health and chronic disease.
Our clinical team are supported well by our excellent management, administrative and reception teams. Many of our employees are long standing and have worked with us for years, a sign that we will look after you!
How will we support you?
The following are the core responsibilities of the ANP indelivering health services.
a. Develop,implement and embed health promotion and wellbeing programmes
b. Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice
c. Implementand evaluate individual specialised treatment plans for chronic diseasepatients
d. Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health
e. Provideadvanced, specialist nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF
f.Be aware of duties and responsibilitiesregarding current legislation and adhere to practice policies and procedures onSafeguarding Adults and Safeguarding Children
g. Requestand collect pathological specimens. Upon receipt, to process and interpretpathology and other test results as required
h. Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required
i.Maintain accurate clinical records inconjunction with extant legislation ensuring that SNOMED CT codes are usedeffectively
j.Maintain chronic disease registers and implementand embed an effective call/recall system
k. Develop,implement and embed well woman and well man clinics
l.Chaperone patients where necessary
m. Prioritisehealth issues and intervene appropriately
n. Supportthe team in dealing with clinical emergencies
o. Recognise,assess and refer patients presenting with mental health needs
p. Supportin the provision of vaccination programmes for both adults and children
q. Bean extended and supplementary prescriber, adhering to extant guidance
r.Support patients in the use of their prescribedmedicines or over the counter medicines (within own scope of practice),reviewing annually as required
s.Contribute to practice targets (QOF, etc.),complying with local and regional guidance
t.Liaise with external services/agencies to ensurethe patient is supported appropriately (vulnerable patients, etc.)
u. Delegateclinical responsibilities appropriately (ensuring safe practice and that the taskis within the scope of practice of the individual)
v. Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
w. Deliveropportunistic health promotion where appropriate
There may be, on occasion, a requirement to carry out othertasks, this will be dependent upon factors such as workload and staffing levels.
Wider responsibilities include;
In addition to the primary responsibilities, the ANP has thefollowing wider responsibilities:
a. Provideholistic support in the clinical governance spectrum assuming a senior role inits compliance
b. Supportthe IPC lead for the practice, ensuring compliance
c. Monitorand ensure the safe storage, rotation and disposal of medications
d. Undertakeoverall management of the nursing team, providing guidance when necessary,acting as a mentor to students and newly qualified staff
e. Participatein local initiatives to enhance service delivery and patient care
f.Support and participate in shared learningwithin the organisation
g. Developan area of specialist interest, taking the lead within the organisation
h. Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate
i.Drive the development of nursing services withinthe organisation, liaising with external agencies and professionalorganisations as required
j.Develop practice administrative and clinicalprotocols in line with the needs of the patient and current legislation
