KeyResponsibilities
ClinicalDuties
- Assess, diagnose, plan, and manage care for patientspresenting with undifferentiated and undiagnosed conditions.
- Deliver same-day/urgent care consultations, managingacute presentations and minor illness.
- Lead and support in long-term condition management(e.g., diabetes, hypertension, asthma), aligned with QOF and local healthpriorities.
- Conduct telephone and face-to-face triage, ensuringappropriate prioritisation of care.
- Prescribe safely and appropriately within scope ofpractice.
- Order diagnostic investigations, interpret results,and make referrals as needed, following local guidelines.
- Participate in and support delivery of specialisedclinics (e.g., diabetes, paediatrics) and home visiting when required.
- Work as an integral part of the multidisciplinaryteam, including GPs, nurses, healthcare assistants, pharmacists, socialprescribers, and mental health professionals.
