A leading healthcare agency is seeking an Advanced Nurse Practitioner to join their team in Barking & Dagenham. The role involves delivering high-quality nursing services, managing chronic disease patients, and developing health promotion programs. Opportunity available for an Advanced Nurse Practitioner to work across Primary Care Services, based in Barking & Dagenham.
The team operates within a Primary Care Service, with the role primarily responsible for delivering evidence-based nursing services as part of a multidisciplinary team. You will provide care within your scope of practice to the entitled patient population, working autonomously. The Nurse Prescriber will handle various clinical areas such as health promotion, chronic disease management, health prevention, well-man clinics, and support the management team in reviewing clinical policies and procedures.
You will have access to appropriate clinical supervision and a designated individual within the organization for daily advice and support. Your responsibilities include developing, implementing, and evaluating health promotion and wellbeing programs, as well as creating individual treatment plans for both acute and non-acute conditions. You will support the clinical team in managing chronic disease patients and identify, manage, and support patients at risk of developing long-term conditions, aiming to prevent adverse health effects.
Providing routine nursing care in accordance with clinical evidence, NICE guidelines, and NSF standards is essential. You will be able to prescribe medication within your scope of practice according to PGD, review medication effectiveness, and adjust treatment as needed.
The hours for this role are full-time - 37.5 hours per week.
