Advanced Nurse Practitioner

Opportunity available for an Advanced Nurse Practitioner to work across Primary Care Services, based in Merseyside and surrounding areas.

The team is part of a Primary Care Service, with the role primarily responsible for delivering evidence-based nursing services as part of a multidisciplinary team. You will work autonomously, managing various clinical areas such as health promotion, chronic disease management, health prevention, and well man clinics. Additionally, you will support the management team in reviewing clinical policies and procedures.

You will have access to clinical supervision and a designated individual within the organization for advice and support. Your responsibilities include developing and implementing health promotion and wellbeing programs, creating and evaluating treatment plans for acute and non-acute conditions, and supporting patients at risk of long-term conditions to prevent adverse health effects.

Providing routine nursing care in accordance with clinical evidence, NICE guidelines, and NSF is essential. You will also be authorized to prescribe medication within your scope of practice according to PGD, review medication effectiveness, and adjust treatment as needed.

The hours for this role are Full Time

The hourly rate for this role:

• Monday to Friday: £47.00 per hour
• Saturday/Sunday: £50.00 per hour
